Abstract:

Introduction: Acute pancreatitis is an acute inflammation of the prior normal gland parenchyma which is usually reversible with raised pancreatic enzyme levels in blood and urine. Severe acute pancreatitis is usually a result of pancreatic glandular necrosis. Necrosis when associated with infection in acute pancreatitis results in higher morbidity and mortality ranging from 10 to 85% .So laparotomy and debridement of the necrosed tissue have been considered a gold standard treatment .Obstruction associated with stones and alcohol abuse are the most common causes of acute pancreatitis

Aims and Objectives: To elaborate the diagnostic approach and management strategy. To explore the potentials of surgical intervention as a life saving measure in acute pancreatitis .To identifiy the prognostic factors and insights the determine the outcome

Methodology: Case presentation: A 24?year?old male presented with abdominal distension persisting since 1 month and breathing difficulty since 20 days. ØAbdominal distention was gradually increasing and was associated with loss of appetite & breathing difficulty with no history of nausea , vomiting ,diarrhea , diabetes , hypertension or tuberculosis. Patient was non- alcoholic , non-smoker and no substance use.

Differential Diagnosis: Perforated duodenal ulcer , mesenteric ischemia , acute cholecystitis and bowel obstruction

Conclusion: The effective management of acute necrotizing pancreatitis in the above performed case marks the crucial role of prompt intervention and comprehensive care . With timely surgical intervention i.e pancreatic necrosectomy was done , the patient’s outcome was favourable
